A comprehensive case management unit that screens all KHCC participants for unaddressed needs — and connects them to the resources, support, and stability they deserve.

KHCConnect services are diverse and varied to meet the full range of community needs, but unified by one purpose: helping families in the Northwest Bronx thrive. By providing a coordinated, trusted point of contact, we help families navigate complex systems, access critical benefits, and build lasting stability.

How We Help

Intensive Case Management

Dedicated, ongoing support to help families navigate complex systems and resolve barriers to stability.

Housing Navigation

Guidance through the housing search and application process, including emergency support for rental arrears.

Crisis Intervention

Immediate support for families in acute crisis, including safety planning and rapid connection to services.

Benefits Assistance & Advocacy

Help applying for and maintaining public benefits, Medicaid recertification, and advocating for families' rights.

New Mother Support Kits

Emergency supplies for new mothers including baby food, formula, clothing, and diapers.

Educational Workshops

Skill-building workshops and educational opportunities designed to strengthen families from the inside out.

Ongoing Counseling

Consistent, supportive counseling to help individuals and families process challenges and build resilience.

Internal & External Referrals

Warm referrals to KHCC programs and trusted community partners to address the full spectrum of family needs.

SNAP Benefits Assistance

Help applying for SNAP benefits and support with benefit and Medicaid recertification.

Housing Connect

Assistance applying for affordable housing through NYC Housing Connect. KHCC is a certified NYC Housing Ambassador.

Free Weekly Food Boxes

Our Food Pantry provides free weekly food boxes to community members in need — open twice a week.

Intimate Partner Violence Support

Crisis intervention, safety planning, and referrals for those impacted by intimate partner violence.

Without accessible, trusted guidance, families spend countless hours trying to secure basic resources, often falling through the gaps between disconnected service systems.
